Description (Service) | COVID-19: Niagara Region Community Services has enhanced the Niagara Gatekeepers program to help reduce isolation and ensure basic needs are addressed. Services are available to those 55 years and older, and include * Weekly, friendly telephone calls to check-in with seniors * Providing access to groceries, medications, personal care items and pet supplies * Ensuring transportation is available for health-related appointments * Supporting access to financial entitlements such as Canada Pension Plan, Guaranteed Income Supplement, etc. Phone line referral service that the public can call to help connect an at-risk senior with programs and services to help them stay safe and independent at home. Referrals will be forwarded to one of the partner agencies. The role of the partner agency is to contact the senior and help get services in place that are intended to help seniors remain safe and independent at home for as long as possible. All information gathered during the calls is confidential and callers may remain anonymous. Services are only provided with the consent of the senior. Partner agencies include: * Niagara Region, Seniors Community Programs * HNHB Local Health Integration Network (HNHB LHIN) – Home and Community Care * Alzheimer Society of Niagara Region * Niagara Region, Mental Health Community education presentations are also offered to any interested group. |
