Description (Service) A prevention/early intervention program which is committed to developing an integrated system of services for children 0-6 years. All new mothers receive a phone call within 48 hours of discharge from hospital and are offered a home visit if additional support is needed. Program provides screening, assessment, and home visiting support from nurses and family home visitors for families who need additional support. Early identification of infant and child development is an integral part of the program and is accomplished through parental screening and assessments by professionals in the community. Referrals and linkages to community partners are made as needed. Parenting workshops and breastfeeding clinics are available.
