| Description (Service) | Provides direct consultation by a home visit team to improve care of persons with late onset mental health problems or behavioural disturbances. The team (geriatric psychiatrist and case manager) assesses the older person in their place of residence. Recommendations are provided to the family physician and follow-up is provided as required by the team. In addition, education and support for health care professionals and caregivers is provided. |
