Description (Service) COVID-19: Hospice, programs and services continue to be offered. In-person programs are being offered virtually and there are safety protocols for the hospice residence. The administrative office is open weekdays from 9 am-5 pm with limited onsite staff. Visit website for details.  
Helps people and families from the time of diagnosis with an incurable condition, during treatment, through to end-of-life care and bereavement. Focuses on improving quality of life in a way that is personal and meaningful to people and families.  
Programs and services are free and are offered in homes, in the community and in hospice residence. 
VISITING VOLUNTEERS: Trained volunteers provide non-medical support and comfort in-home for people living with an incurable condition. 
DAY HOSPICE: People living with an incurable condition enjoy a day of caring, friendship, entertainment and relaxation in a comfortable setting, while caregivers have a day of respite. Offered in St. Catharines and Welland, with French language option. 
HOSPICE RESIDENCE: Our 10-bed residence in St. Catharines provides quality hospice palliative care in a home-like setting for residents of Niagara. 
COMMUNITY PALLIATIVE CARE OUTREACH TEAMS: Teams work in partnership with primary care providers, including family physicians and visiting nurses, to provide clients with consistent and comprehensive care in their home. 
PALLIATIVE PAIN AND SYMPTOM MANAGEMENT CONSULTATION PROGRAM: Palliative pain and symptom management consultation and interdisciplinary palliative care education to Niagara health care professionals.  
BEREAVEMENT SUPPORT PROGRAMS: Programs for children, adolescents and adults who are grieving a death. Program is not intended to replace one-on-one professional counselling.

Eligibility Anyone with an incurable condition. Parental consent is required for youths under 18 years accessing bereavement programs. Contact office for details.
