| Description (Service) | Offers patients a team approach to health care. The team includes patients, family doctors, social workers, a physician assistant, registered nurses, dietitians, a diabetes education team (a certified diabetes educator nurse and dietitian), nurse practitioners, respiratory therapist and a psychiatrist plus a consultant in sports medicine. Programs include: an After Hours Urgent Care Clinic; a Stop Smoking Clinic; Asthma & COPD Clinic; Mental Health Assessments; Nutritional & Lifestyle Counselling; Diabetes Education and Teaching; Memory Clinic; Health Care and Education; Craving Change Healthy Lifestyle Program; Self Management Programs and counselling for a new crisis or an ongoing problem.
