Description (Service) A non-profit, community-governed organization that provides primary health care, health promotion and community development services. Multi-disciplinary teams of health providers consisting of Physicians, Nurse Practitioners, Nurses, Counsellors, a Dietitian, a Health Promoter and Outreach Workers assess and treat non-life threatening injuries or illnesses, and develop programs to improve the social supports and conditions that affect long-term health. 
 
Services include: 
* Primary Health Care Services 
* Counselling 
* Over 25 Health Promotion Programs 
* Outreach support 
* Caring For My COPD program 
* Smoking cessation support - individual and group support
Hours Mon, Wed, Fri 8 am-4:30 pm * Tue 7:30 am-7:30 pm * Thu 7:30 am-7:30 pm

Eligibility Residents of Niagara Falls and individuals without primary care. 
 
NFCHC priority population has been defined as: 
* Individuals/families without a healthcare provider 
* People with mental health and/or addiction issues 
* Low income families 
* Isolated seniors 
* At risk children and youth 
* Street involved/non-insured persons 
* Newcomers to Canada 
* People with physical disabilities

Peer Support Drop-In Group 
Looking for a safe space to get out, meet people and support one another?  
Stop by on Mondays from 10 am-11:30 am  
 
Birth Certificate Clinic 
Lost your birth certificate ID?  
Helps with Birth Certificates process. Subsidy available for Birth certificates. 
Tuesdays 1 pm-3 pm at the Niagara Falls Community Health Centre. 
 
Gentle Fit Drop-In Group 
Tuesdays 10 am-10:45am and Thursdays 1 pm-1:45 pm in the upstairs community room. Everyone is welcome. Call Alison for details. 
 
Coffee Club Drop-In 
Every Thursday from 10 am-11 am in the NFCHC basement for some friendly chat and community information. Call Alison for details.
