|Description (Service)||A non-profit, community-governed organization that provides primary health care, health promotion and community development services. Multi-disciplinary teams of health providers consisting of Physicians, Nurse Practitioners, Nurses, Counsellors, a Dietitian, a Health Promoter and Outreach Workers assess and treat non-life threatening injuries or illnesses, and develop programs to improve the social supports and conditions that affect long-term health.

|Eligibility||Residents of Niagara Falls and individuals without primary care.
NFCHC priority population has been defined as:
* Individuals/families without a healthcare provider
* People with mental health and/or addiction issues
* Low income families
* Isolated seniors
* At risk children and youth
* Street involved/non-insured persons
* Newcomers to Canada
* People with physical disabilities

Lost your birth certificate ID?
Helps with Birth Certificates process. Subsidy available for Birth certificates.

Gentle Fit Drop-In Group
Tue, Thu 1 pm-1:45 pm in the upstairs community room. Everyone is welcome. Call Shelby for details.
Coffee Club Drop-In
Every Thursday from 10 am-11 am in the NFCHC basement for some friendly chat and community information. Call Shelby for details.
1st and 3rd Wed of every month from 9 am-12 noon, 1 pm-3 pm, free drop-in counselling for anyone in the community.
Young & Pregnant
Every Thu from 3:30 pm-5:30pm, a prenatal program for expecting mothers up to the age 21. Call Nicole for details.
