Description & Services
Description (Service) Provides primary healthcare to the residents of Niagara Falls along with health promotion programs and community development services. Patients have access to family physicians, nurse practitioners, registered nurses, a dietitian and social workers. There is also a Health Promotion team that provides community programs and outreach support. Open to the community. 
 
Services include: 
* Primary Health Care Services 
* Counselling 
* Over 25 Health Promotion Programs 
* Outreach support 
* Caring For My COPD program 
* Smoking cessation support - individual and group support
Hours Mon, Fri 8 am-4:30 pm * Tue, Thu 7:30 am-7:30 pm * Wed 8 am-4:30 pm * Walk in Clinic for Registered Patients Sat 9 am-12 noon

Eligibility All residents of Niagara Falls and/or individuals without primary care. 
 
NFCHC priority population has been defined as: 
* Individuals/families without a healthcare provider 
* People with mental health and/or addiction issues 
* Low income families 
* Isolated seniors 
* At risk children and youth 
* Street involved/non-insured persons 
* Newcomers to Canada 
* People with physical disabilities
Application Registration required for some programs. Call for information.

Birth Certificate Clinic 
Lost your birth certificate ID?  
Helps with Birth Certificates process. Subsidy available for Birth certificates. 
Tue 1 pm-3 pm at the Niagara Falls Community Health Centre. Everyone is welcome. 
 
Gentle Fit Drop-In Group 
Tue, Thu 1 pm-1:45 pm in the upstairs community room. Everyone is welcome. Call Shelby for details. 
 
Coffee Club Drop-In 
Every Monday from 1:30-2:30pm at The HUB (4333 Queen St) and every Thursday from 10 am-11 am in the NFCHC basement for some friendly chat and community information. Call Lisa for details. Everyone is welcome. 
 
Young & Pregnant 
Every Thu from 3:30 pm-5:30pm, a prenatal program for expecting mothers up to the age 21. Call Nicole for details. 
 
Youth Programming 
A number of programs for youth/young adults are available as well. Please check our website or social media for a current list of programs.
