|Description (Service)||Program is offered in collaboration with the Center for Healthy Aging/HHSC/St. Peter's Hospital in Hamilton.
Patients must be referred to the program at West Lincoln Memorial Hospital by a Physician. Outpatient referrals are assessed using a comprehensive, standardized assessment of a person's environmental needs, and physical, psycho-social, cognitive and functional abilities, and are completed by a Nurse Clinician with a family member or caregiver present. The individual is then assessed in clinic or virtually by the Geriatrician and the Nurse Clinician. A consultation report is generated for the referring Physician and recommendations for care/tests/services are shared.
Follow up clinic appointments as determined by the Geriatrician. Other Health Professional cross referrals, when appropriate, on a case by case basis.
