| Description (Service) | A free 6-month blood pressure management program. Participants receive access to an online health platform with curated content and trackers, support from a personal health coach and Loblaws in-store Dietitians, a 2-month free membership to participating YMCA-YWCA Health & Fitness Centres (for non-Y members), and PC OPTIMUM points to reward healthy behaviours. |

| Eligibility | Ages: 40 year(s) and up Participants cannot be on blood pressure medication and must have blood pressure between 121-139 systolic or if currently living with diabetes 121-129 systolic (volunteers will measure this for applicants). |
