Description (Service) COVID-19 - In-person groups and classes cancelled until further notice. Some virtual services available.

Provides primary health care services and health promotion programs using a multidisciplinary team of health care providers. Team includes Family Physicians, Nurse Practitioners, Registered Nurses, Registered Dietitians, Diabetes Educators, Social Workers, Health Promoters, Community Health Workers, a Physiotherapist, Physiotherapist Assistant and Kinesiologist.

Diabetes Education Program - Information about living with diabetes * includes monitoring, blood sugar levels, food choices, exercise, medications, and lifestyle choices * available to people with type 1 or type 2 diabetes. Physician or self referrals are welcome.

Physiotherapy Services - For those who do not have benefit coverage (OHIP or work benefits), free physiotherapy services are offered to get on the right track to physical wellbeing, while also promoting self-management and independent exercise. Physician and self referrals welcome.

Fitness Classes - A variety of classes are offered, a different one each day of the week, all free of charge. Space is limited for some classes, so call in to be added to the class list. Classes offered include Nordic Pole Walking, Walk,Talk & Stretch, Gentle Fit, Keep it Moving, Back to Basics. See website and newsletter for more details.
