|Description (Service)||A community based organization mental health housing and support service provider that assists individuals to achieve greater independence through innovative, consumer centered housing and support services.
Operates a residence in the Niagara area. The residence is located in Welland, which has the capacity to accommodate seven tenants. Professional staff provide intense case management and support services. Tenancy costs are based on ability to pay through shelter allowances.
COMMUNITY SUPPORT PROGRAM:
Case managers provide moderate level support to consumers throughout the Niagara Region that include engaging consumers and/or families to provide emotional support and mental health education, daily life counselling and referral for appropriate clinical treatment, legal advocacy regarding rights and entitlements representation, discharge planning from hospital and correctional facilities and community mental heath services discharge planning.
HOMES FOR GOOD:
Homes for good is a supportive housing program for homeless clients with complex needs who require multiple supports. The program offers a multi-disciplinary team including; occupational therapy, addiction support, landlord relations coordinator, and case management. Subsidy provided to help cost of market rent. Target clientele chronically homeless (180 days or more), youth homeless (Ages 16 years and older) , indigenous homeless, homeless following provincially-funded institutions or service system (eg jail, hospital, FACS). All homelessness service providers can submit an application to Niagara Region Homelessness Services using a client eligibility application form.
Housing First Program is a recovery-orientated approach to end homelessness that centers on quickly moving people experiencing homelessness (180 days or more) into independent and permanent housing and then providing additional supports and services as needed.
HOMELESSNESS INITIATIVE PHASE II:
As a partner in the Homelessness Initiative Phase II program Gateway provides a combination of affordable housing and ongoing flexible support to maintain housing for individuals with serious mental health issues, who are homeless or at risk of becoming homeless. Gateway leases units from the private rental market and in turn rents them to consumers in the program. These units are subsidized so that tenants can afford a home that is conducive to health maintenance and recovery.
COMMUNITY TREATMENT ORDERS PROGRAM (CTO):
Provides enhanced case management services that are in partnership with doctors and other community health professionals. CTOs are a mutual agreement by all parties, are consent based and can be revoked at any time by either party.
SUPPORTIVE HOUSING & PROPERTY MANAGEMENT:
Gateway owns and operates six supportive housing buildings in the Niagara Region. The Dr Peter Grant Haven of Hope and Canal view homes is located in Welland, Grundy House located in St Catharine's, Berard located in Niagara Falls, Repei Place and Repei Phase II located in Port Colborne. Property management is also provided to partner organizations in the Niagara Region.
