|Description (Service)||COVID-19: Programs have been temporarily suspended. All active clients are able to receive scheduled wellness telephone calls.
A multi agency partnership located on-site at Niagara College, Welland campus offering a collection of services for seniors.
Services are focused on supporting older adults at risk for hospitalization and/or older adults motivated to stay healthy, safe and strong by providing rehabilitation, wellness programs and community resource connections.
Services are provided through 2 programs: Ambulatory Rehab Program and a 12 week Independent Living/Wellness curriculum. The programs run congruently but have different entry criteria.
