| Description (Service) | An inter-disciplinary team of health care providers who provide coordinated medical, nursing and psycho-spiritual care services as well as bereavement care services to individuals facing life-threatening illness and their family. The team works in consultation with the family physicians, Home and Community Care, community service provider agencies, West Lincoln Memorial hospital, Long term-care facilities and McNally House Hospice. The team travels to meet people in the most appropriate setting of care. |
