Description (Service) | Assists adults with special needs who are dealing with significant behavioural issues. Assistance is provided to the individual and their care providers. Before Community Response Program services can be initiated, the individual must have a local Behaviour Service Provider assigned. Works collaboratively with the local Behaviour Services Provider and relevant professionals to enhance direct treatment and stabilization of presenting behaviours. Offers three categories of clinical support: 1. Outreach: In-Home Support is provided for an individual who is living at home with his/her family or in a home operated by a transfer payment agency. A Community Response Program team member (Resource Specialist) will provide observation, programming recommendations, staff/family training, and hands-on implementation of any behavioural programming set forth by the local Behavioural Service Provider. 2. Stabilization: Short-term access to additional funding or a stabilization bed when an individual requires stabilization outside of their home. 3. Residential Treatment: A fully-funded six-bed residential treatment home located in Vineland. Individual treatment planning occurs on site under the direction of a clinical support team and qualified staff. Treatment typically continues for a period of 90 days up to two years. Referring agencies/parties will contribute by attending case conferences and have regular interaction with Bethesda’s clinical team. Transition and discharge planning will begin on admission and develop with the discharge community. |
