Description (Service) | Residential care to people who need time and support to recover strength, endurance, and functioning before returning home from a visit to a hospital * a range of services are provided by a team of on-site professionals including family physicians, nurse practitioners, nurses, personal support workers, physical and occupational therapists, and social workers Program is best suited for people who are motivated and capable of returning home within 90 days Client must stay a minimum of seven days to a maximum of 90 days per year * The expected length of stay in the program is between 30-45 days A partnership involving Niagara Region Senior Services, Linhaven long-term care home, Hamilton, Niagara, Haldimand, Brant Home and Community Care, Ministry of Health and Long Term Care, Hamilton Niagara Haldimand Brant Local Health Integration Network |
Hours | Office Mon-Fri 8:30 am-4:30 pm * Service Mon-Sun 24 hours |
Eligibility | Adults 18 years and older who require on-site nursing care 24 hours a day, assistance with activities of daily living, or monitoring throughout the day * program not intended for individuals waiting for admission to a long-term care home, requiring complex continuing care or palliative care Open to all adults who have Health Insurance through the Ontario Health Insurance Plan (OHIP) |
Application | All applications to the program are processed by the local HNHB Local Health Integration Network (HNHB LHIN) – Home and Community Care, which also: * Determines eligibility for the program * Assists in developing care plans * Manages the admissions process * Facilitates program discharge plans Note that pre-booking is not available. |
