Description (Service) A non-profit organization dedicated to assisting and responding to the challenge to caregivers of finding loved ones who due to their cognitive ability have a tendency to wander * participants wear a bracelet transmitter that emits a tracking signal once every second
Hours Service Mon-Sun 24 hours
Eligibility An individual of any age who is inclined to wander or become 'lost' related to a cognitive condition. Must be willing to wear a bracelet 24/7.
Application Family caregivers, or agency referral with family consent, can apply for the program.  
Copies of documentation of Doctor’s assessment, legal guardianship, and/or birth certificate * Will need to provide the attending physician’s name and phone number and a list of all medications the participant is currently taking.

Fees Service - Initial participants startup fee: $149 with $100 refundable upon return of undamaged radio wrist transmitter and battery tester * Monthly appointment maintenance fee: $25.
