| Description (Service) | COVID-19: Service will be provided primarily via telephone. In-person visits are also available on a case-by-case basis. Niagara Region offers a range of outreach services to older adults (60+) to help them stay safe and in their own homes for as long as possible. Community workers provide in-home visits to determine what the senior wants and needs to stay at home. Staff can make referrals on the senior's behalf to programs designed to improve safety, independence and quality of life including: * Fall prevention through exercise (Healthy, Safe, Strong) * Adult Day Service * Respite Companion Program * Affordable, appropriate housing * Meal services * Specialized services such as Alzheimer Society and mental health services * Transportation services |
