Description (Service) | Designed to assist clients and their families who are entering, involved with or exiting the Niagara Health System . Through a Wholistic Approach the Aboriginal Patient Navigator will provide services that are culturally appropriate to assist individuals and their families to better understand and utilize the Niagara Health System. Services Include: Individualized Cultural Support * Coordination of Services * Advocacy * Hospital Visits * Discharge Planning * Referrals and Linkages to Community Resources * Access to Traditional Healing Programs * Home Visits * Community Follow-up The APN program will help individuals, care-givers and their families to: better understand their plan of care and treatment options, overcome barriers while in the hospital and in the community, improve communication with service providers, as well as increase a greater sense of physical, emotional, mental, and spiritual support. |
