| Description (Service) | Formerly Community Care Access Centre Access to health care support at home and in the community. Care coordinators work with people who are going home from the hospital and those in the community to help people access the care and services they need. Care is provided in community settings such as LHIN Nursing Care Centres, at home, school, residential facility or LHIN Nursing Care Centre. When appropriate, referrals are made to other community services available which may include: • nursing • nurse practitioner • personal support • physiotherapy and occupational therapy • speech and language therapy • social work • nutrition and food counselling (dietitians) • end-of-life care • medical supplies and equipment (patient-specific supplies for CCAC care, such as catheters, ostomy bags, or wound care supplies) • access to long-term care homes • access to supported living (adult day programs, supportive housing, retirement homes, and assisted living) • health support in the school • specialized services for people living with specific health conditions • referral to other community supports Information and Referral – thehealthline.ca provides online access to health care information and services in Ontario. | Eligibility | People of all ages may receive services. You must be a resident of Ontario, have a valid Ontario health card, be able to receive care safely at home. Some clients may receive care in other settings such as at school or at a community based nursing clinic. Clients are assessed by Home and Community Care staff using guidelines provided by the Ministry of Health and Long-Term Care. School Health Support Services are available province wide to children 21 years of age or younger with special needs who are registered with the Ontario Ministry of Education. Children must have a valid Health Card and be residents of Ontario needing special health services in the school setting. Children must attend a publicly funded or private school or receive satisfactory instruction in the home. Additional eligibility criteria will be assessed upon initial intake. |
