| Description (Service) | A 10 week, community-based program designed to enhance the quality of life for people with Chronic Obstructive Pulmonary Disease (COPD). Program participants will have access to: * Group and individual COPD education by qualified health care professionals * Peer Support * Family and caregiver educational sessions * Personalized exercise programs * Smoking cessation counselling and support * Transportation to program if needed * Regular communication with family physician and respirologist * Ongoing support for 1 year after the program ends * Personal counselling and spiritual support |
