| Description (Service) | COVID-19: Programs have been temporarily suspended. All active clients are able to receive scheduled wellness telephone calls. A structured 6-8 week rehabilitation program for seniors experiencing problems with mobility (falls, decreased strength and/or balance, low stamina). The program aims to assist participants in reaching optimal mobility function and independence. Participants receive a treatment plan based on identified goals that includes appointments with the Rehab Team and participation in the 12 week Independent Living/Wellness curriculum. Participants are also connected to additional community resources to further support their independence after completion. |

| Eligibility | Ages: 55 year(s) and up Older adults recently discharged from hospital or at risk of hospitalization due to frequent falls/general physical decline. Able to transfer without assist or with one person assist. Eligible participants will also: * Have a written medical referral * Be medically stable * Have potential to improve functionally * Demonstrate motivation and willingness to participate |
