Description (Service) | A structured 6-8 week rehabilitation program. The program aims to assist participants in reaching optimal mobility function and independence. Participants receive a treatment plan based on identified goals that includes appointments with the Rehab Team and participation in the 12 week Independent Living/Wellness curriculum. Participants are also connected to additional community resources to further support their independence after completion. |
Hours | Mon-Fri 9 am-12 noon, 1 pm-4 pm |
Eligibility | Ages: 55 year(s) and up Older adults recently discharged from hospital or at risk of hospitalization due to frequent falls/general physical decline. Able to transfer without assist or with one person assist. Eligible participants will also: *Have a written medical referral *Be medically stable *Have potential to improve functionally *Demonstrate motivation and willingness to participate |
