|Description (Service)||COVID-19: Programs have been temporarily suspended. All active clients are able to receive scheduled wellness telephone calls.
A revolving 12 week Independent Living/Wellness curriculum for older adults focusing on four main themes: Promoting Health and Preventing Disease and Injury, Optimizing Mental and Physical Function, Managing Chronic Conditions and Engaging with Life. Participation in the curriculum is included in the Ambulatory Rehab Program.

|Eligibility||Ages: 55 year(s) and up
Older adults experiencing a decline in physical function and/or are inactive; are motivated to stay healthy, safe and strong. Participants must be able to transfer independently or with one person assist.
|Application||Self and agency referrals accepted, call for information.
*Physician's referral needed to receive physiotherapy services (for those experiencing a high volume of falls, at risk of hospitalization or recently discharged from hospital).
