Description (Service) | COVID-19: Programs have been temporarily suspended. All active clients are able to receive scheduled wellness telephone calls. A revolving 12 week Independent Living/Wellness curriculum for older adults focusing on four main themes: Promoting Health and Preventing Disease and Injury, Optimizing Mental and Physical Function, Managing Chronic Conditions and Engaging with Life. Participation in the curriculum is included in the Ambulatory Rehab Program. Topics include: * The Importance of Physical Activity * Fall Prevention * Foot Care * Managing Mental Health * Community Engagement * Meaningful Activities * Goal Setting * Nutrition * Medication Safety * Sleep Hygiene * Arthritis * Diabetes * Heart Health * Osteoporosis * Lifelong Learning |
Hours | Mon-Fri 9 am-12 noon, 1 pm-4 pm |
Eligibility | Ages: 55 year(s) and up Older adults experiencing a decline in physical function and/or are inactive; are motivated to stay healthy, safe and strong. Participants must be able to transfer independently or with one person assist. |
Application | Self and agency referrals accepted, call for information. *Physician's referral needed to receive physiotherapy services (for those experiencing a high volume of falls, at risk of hospitalization or recently discharged from hospital). |
