DescriptionThe Beamsville Medical Centre Family Health Team offers patients a team approach to health care. The team includes patients, family doctors, nurses, social worker and family therapist, dietitians, nurse practitioners, and clinical pharmacist. 
 
Programs include: an Asthma program, a Hyptertension program, Weight Management, High Cholesterol and Blood Lipids, Diabetes Prevention/Pre-Diabetes, DASH Diet Groups, Stress Management, Mind Over Mood, Osteoporosis Management, Smoking Cessation, Diabetic Consult, Epipen or Twinject Use and Demo, Medication Reviews, Falls Prevention, Foot Care program and Arthritis Rehabilitation and Education Program.
HoursMon-Thu 9 am-8 pm, Fri 9 am-5 pm
