| Description | The Garden City Family Health Team offers patients a team approach to health care. The team includes patients, family doctors, social workers, physician assistants, registered nurses, dietitians, nurse diabetes educators, dietitian diabetes educators, nurse practitioners, respirologist and psychiatrists plus consultant Sports Medicine doctor.
Programs include: an After Hours Urgent Care Clinic; a Stop Smoking Clinic; Asthma & COPD Clinic; Mental Health Assessments; Nutritional & Lifestyle Counselling; Memory Clinic; Health Care and Education; Craving Change Healthy Lifestyle Program and counselling for a new crisis or an ongoing problem. |
