DescriptionCOMMUNITY SUPPORT PROGRAM program provides support to people who have serious, persistent mental illness, who are living in the community in the setting of their choice and who at times, require support to maintain their housing and independence. This program provides low to moderate intensity support based on individual needs. 
 
PHASE II HOUSING PROGRAM program offers subsidized supportive housing to individuals with diagnosed serious persistent mental illness and a history of homelessness. The program offers low to moderate support. 
 
TRANSITIONAL HOUSING SUPPORT PROGRAM Glendale: A four bedroom male home, located in St Catharines and Stamford: A seven bedroom home for females located in Niagara Falls.  
 
Criteria for Transitional Housing - Individual has history of chronic homelessness, takes action if there are substance abuse issues, agreeable to programming, able to live in communal environment without 24 hour support. The individual and group therapeutic support, life skills instruction and practice are an integral part of the residential program. The program is time limited to a maximum of 12 months. Assistance with finding permanent housing is a component of this program.

FeesNone ; No fees for support. There are rental fees for Phase II and Transitional, but they are subsidized.
