DescriptionThe Family Health Team uses a group-approach to primary health care, which includes 10 physicians, nurses and nurse practitioners, pediatric and adult psychiatrists, a mental health coordinator and nurse, a dietitian, a social worker, a chiropodist, a pharmacist and a geriatrician. Offers several programs and groups to patients of the practice, including mental health, diet and nutrition and diabetes support.
HoursMon-Fri 9 am-12 noon, 2 pm-4:45 pm

EligibilityMust not be already rostered with one of the 10 physicians at the Niagara-on-the-Lake Family Health Team. 
Must live in Niagara-on-the-Lake, Virgil, St Davids or Queenston and not have a doctor. 
If a person does not live in the above mentioned towns, but works full-time in Niagara-on-the-Lake, they may be eligible for service.
