Description	Integrated standardized foot care assessment, education and treatment program in dialysis centres including nail care, chiropody services, and home services for qualifying seniors with renal insufficiency and diabetes or who have been assessed as not being able to adequately perform self foot exams.

Eligibility	Ages: 55 year(s) and up    Seniors, who have been diagnosed with diabetes or kidney failure and are unable to adequately perform foot exams (as there is a higher risk of foot ulcer with renal failure and diabetes).
