DescriptionA multi-disciplinary agency serving the French speaking population of Niagara and Hamilton Regions.  
 
PRIMARY HEALTH CARE SERVICES: Health care professionals include: Family physicians, clinical care, registered nurses, Lab services, Occupational therapist - Vaccinations - Foot care - Diabetes education and management program. Nutrition: Individual counseling from a registered dietitian. Educational workshops providing practical and reliable information and resources. Senior's services Program: Blood pressure clinics - support services and programs for seniors 55 and older -Home visits from a health professional - Case management services for French-speaking seniors or frail adults - Visiting nurse services may be available for eligible clients - Health promotion workshops -Social and intergenerational activities - physical activity sessions and other community events; "Soyons actifs" a weekly exercise program available to seniors -Volunteer opportunities. 
 
COMMUNITY CONNECTIONS MENTORSHIP PROGRAM (formerly HOST Program) -- group activities for newly arrived immigrants and refugees of all ages to help orient them to their new community and establish social and professional networks * trained and experienced volunteer mentors assist, with focus on citizenship and civic engagement or skills and career development 
 
COMMUNITY HEALTH: Health promotion programs include : Oral Health Workshops and dental hygiene services through a partnership with the Niagara Region for eligible individuals -Healthy Smiles Ontario Program. Jeunesse en mouvement (JEM) - Activity program for youth (6-10 years) - O...! ma santé Activity program for children (2-6 years) 
Community Action Program for Children (CAPC) and Canada Prenatal Nutrition Program (CPNP)- Volunteer opportunities. 
 
COUNSELLING AND COMMUNITY INITIATIVES 
Children's Mental Health Program: A wide variety of services are available to assist children, youth and families coping with mild to moderate mental health issues. Individualized assessment, counselling, case management and follow-up services are available to French-speaking children who have been exposed both directly and indirectly to some form of trauma such as violence, abuse, intimidation, war or torture.  
The Service Coordination Program provides assistance to children and youth who require specialized mental health services that are currently not available in French. Services in this program may include: accompaniments and interpretation services, assisting English-speaking mental health professionals in the development of case plans for French-speaking clients, carrying out client assessments, counselling/treatment and case management services as agreed upon and in collaboration with the English-speaking service provider.  
Note: All service requests for the children's mental health program must be made through Contact Niagara at 905-684-3407 or 1-800-933-3617. 
 
Adult counselling services: Counselling services for those facing issues such as depression, bereavement, relationship difficulties with children etc. The CSC provides counselling and transitional support services for women who have been or are currently victims of abuse. Practical assistance such as accompaniments to lawyers' appointments, referrals to shelters or other housing options and safety planning are available. Off site services may be available to abusive partners through the Partner Abuse Response (PAR) program. The CSC also partners with a 24 hour French Language Crisis line for victims of woman abuse "Femaide" 1-877-336-2433.  
 
SERVICES FOR FAMILIES WITH YOUNG CHILDREN:  
Pre/Post natal program (CPNP): information on pregnancy, nutrition, labour, referrals to midwifery services, care of newborns, breastfeeding support, home visits, vouchers for vitamins, groceries and diapers.  
 
The CSC Hamilton/Niagara offers a Parent resource program Grandir ensemble - Education and support for parents of children 0 to 6 . Helping families grow and learn in a happy, healthy, safe environment and allowing children to participate in various activities in the area of sciences, literacy, creativity, etc.  
 
L'école j'en raffole (School's Cool - school readiness program) - Summer camps  
Community lunches and dinners with parenting workshops - Early Years Centres - Community and in-house support - Parenting and other educational workshops 
Toy lending library - Best Start wrap around services (early screening and intervention) 
Inter-generational programming combining geriatrics and early years needs - Pre-school speech and language services are available through partnerships with local agencies. 
 
SETTLEMENT PROGRAM (formerly Immigrant Settlement and Adaptation Program): In collaboration with local partners, the CSC offers a wide range of services to enable immigrants and refugees to integrate into Canadian society including orientation programs, language and skills training to help facilitate employment, and programs to welcome newcomers to the community * individual agencies may provide needs and eligibility assessments, pre- and post-arrival information and referral, job search and employment counselling, assistance to establish social and professional networks to promote community engagement, and assistance to access a range of settlement services 
 
Settlement workers assist newcomers in the following areas: Settlement services (intake, needs assessments, help with housing, Ontario Works, immigration and other government procedures) * settlement and orientation information * community legal services * informative workshops * referrals to community services * Interpretation and outreach assistance * in-school support for parents and families * Informative workshops and other activities.
