DescriptionFocuses on caring support and quality of life for seriously ill people residing in the town of Niagara-on-the-Lake and their families. Offers services at home, at the three long term care facilities or at the local hospital through trained visiting volunteers. Provides compassionate, emotional support, companionship and caregiver relief. Lends equipment such as eggcrate mattresses, wheelchairs, recliner/lift chairs, walkers, TV/DVD players and relevant books, CD's and DVD's. Also provides phone and group bereavement support and overnight accommodation for clients' out-of-town family members. Everything is free-of-charge and with strict confidentiality.

EligibilityAny resident of the town of Niagara-on-the-Lake, including individuals in a long-term care facility, the hospital or a private home.

ApplicationAnyone can refer - a medical referral is not required. The Program Coordinator will assess the needs of the clients and their families or caregivers and suggest ways to help them cope.
