DescriptionThis program is offered in collaboration with the Center for Healthy Aging-HHS-St Peters Hospital in Hamilton.  
 
Inpatients must be referred to the program at West Lincoln Memorial Hospital by a physician. They are initially assessed by a Nurse Clinician. This involves a comprehensive standardized assessment of a person's physical, psycho-social, cognitive,functional,environmental needs and abilities. The patient is then seen by a Geriatrician at a later date. 
 
Outpatient referrals are assessed as above, and are done by a Nurse Clinician at the person's place of residence, with a family member or caregiver present if possible. The patient would proceed to the outpatient clinic at a later date for a geriatric assessment appointment with the Geriatrician. A consultation report is generated for the referring Physician and recommendations for care/tests/services are shared.  
 
Follow up clinic appointments as determined by the Geriatrician. Other health professional cross referrals are offered, when appropriate, on a case by case basis.
