| Description | Community Health Centres (CHCs) are non-profit, community-governed organizations that provide primary health care, health promotion and community development services, using multi-disciplinary teams of health providers. These teams often include physicians, nurse practitioners, dietitians, health promoters, counsellors and others who are paid by salary, rather than through a fee-for-service system. Community Health Centres are sponsored and managed by incorporated non-profit community boards made up of members of the community and others who provide health and social services. Services are designed to meet the specific needs of a defined community. In addition, CHCs provide a variety of health promotion and illness prevention services which focus on addressing and raising awareness of the broader determinants of health such as employment, education, environment, social isolation and poverty. Groups: Strengthening Families A 14 week prevention program for families with children between the ages of 7 and 11 who may be at risk for substance use, depression, violence & school failure. Chronic Disease Self Management A 6-week workshop for people who live with an ongoing health problem who need help identifying coping strategies, setting goals, managing stress, making healthier food choices, working with their healthcare team and living life to the fullest. Call Julia at x 297 for more information. Craving Change This how-to workshop is focused on changing your relationship with food. You will learn to understand why you eat the way you do; how to comfort yourself without food and to change your thinking so you can change your eating. Call Amy at x 234 for more information. Gender Quest A support group for people in any stage of gender transition, modeled after Sherbourne Health Centre’s Gender Journeys group. Call Stephanie at x 222 for more information. Healthy Choices for Recovery 1 A group for discussing addiction issues and strategies for making healthier choices to maintain abstinence, using a harm reduction approach.
