DescriptionBridges Community Health Centre provides primary health care services and health promotion programs using a multidisciplinary team of health care providers. Our team includes Family Physicians, Nurse Practitioners, Registered Nurses, Registered Dietitians, Diabetes Educators, Social Workers, Consulting Psychiatrist, Health Promoters and Community Health Workers. 
 
Diabetes Education program is available to anyone with Type 1 or Type 2 Diabetes. 
 
Mental Health Services include mental health assessments, short term supportive counselling, linkages with community services and psychiatric consultation regarding treatment, diagnosis and medication. Referrals to consulting psychiatrist are through a physician referral. 
 
This is a site location for the Good Food Box in Fort Erie.
