DescriptionOut-patient services including the following:  
 
Chronic Kidney Disease Program -- provides hemodialysis treatments, peritoneal dialysis education/support and renal insufficiency/pre-dialysis counselling and monitoring to delay patients from requiring dialysis services. Contact 905-687-3712, Monday-Saturday 6:30 am-11 pm. 
 
Cardiac Health and Rehab Program -- offers a variety of cardiovascular rehabilitation and risk reduction services such as; supervised exercise programs, nutrition counselling and stress management. Contact 905-641-2542, Monday-Friday 8:30 am-4:30 pm. 
 
Niagara Diabetes Centre, St Catharines satellite location -- provides diabetes counselling/education either in group or individual appointment formats for newly diagnosed and established diabetes. Specialty programs are also offered for paediatrics, pregnancy and individuals on multiple insulin injections or insulin pumps and smoking cessation. All appointments are booked through the main office in Welland using Phone 905-682-4200 or Fax 905-682-3622. Program access requires physician referral. General information can be accessed via NDC@niagarahealth.on.ca. 
 
Volunteer Resources -- More than 1100 Volunteers support the work of NHS Hospitals with patient care, fundraising, and customer service roles. Interested applicants can learn more and apply online at www.niagarahealth.on.ca or call ext. 44205 
 
Hepatitis C Clinic, Satellite Site -- Services offered include treatment for Hepatitis C infection, case management and individual and group counselling. Patients can be referred by a family physician, Niagara Region Public Health or community health centres. Individuals who know they have Hepatitis C or believe they may be at risk of infection can also self refer by contacting 905-378-4647 ext 32554
