| Description | The Niagara West Palliative Care Team consists of an inter-disciplinary team of health care providers who provide coordinated medical, nursing and psycho-spiritual care services as well as bereavement care services to individuals facing life-threatening illness and their family. The team works in consultation with the family physicians, CCAC, community service provider agencies, West Lincoln Memorial hospital, Long term-care facilities and McNally House Hospice. The team travels to meet people in the most appropriate setting of care. The goal is to provide the best possible quality of life for patients and their families. |
