| Description | In-patient and Out-patient services: General Medical/Surgical; Complex Continuing Care (Chronic Care); Geriatric Medicine; Mental Health; Emergency; Intensive Care; Ophthalmology; Ear, Nose and Throat; Gynaecology; Orthopaedics; Plastic Surgery; Urology; Dental; Palliative Care; Laboratory; Diagnostic Services, Rehabilitation Services; Discharge Planning and Quick Response Team. Outpatient clinics: Ontario Breast Screening Program -- a breast screening and assessment service for women 50 years of age and older. No referral necessary but appointment required. Contact 905-358-4952. Breastfeeding Clinic -- provides access to certified lactation consultants offering support, advice and encouragement to lactating women and their children. Contact 905-378-4647 ext 53705. Cardiac Health and Rehab Program -- offers a variety of cardiovascular rehabilitation and risk reduction services such as; supervised exercise programs, nutrition counselling and stress management. Contact 905-641-2542. Niagara Diabetes Centre, Niagara Falls satellite location -- provides diabetes counselling/education either in group or individual appointment formats for newly diagnosed and established diabetes. Specialty programs are also offered for paediatrics, pregnancy and insulin pumps. All appointments are booked through the main office in Welland via phone 905-682-4200 or Fax 905-682-3622. Niagara District Stroke Centre -- provides a range of programs for Niagara residents, including partnerships for comprehensive stroke care with Niagara EMS, ER, Inpatient Acute Stroke Unit, follow-up care, Stroke Prevention Clinic and more. Contact 905-378-4647 ext. 55556. Geriatric Assessment Outpatient Clinic -- provides a comprehensive physical, psychosocial and environmental needs assessment for Niagara's senior population. Speech Language Pathology Department -- Provides Speech Language Pathology service to preschoolers (0-5 yrs) through Speech Services Niagara and adults (18 yrs and up) on an in-patient and out-patient basis.
