Works closely with the Hamilton, Niagara, Haldimand Norfolk Brant and Burlington Palliative Care Consultation Programs.  
 
Improves access to * the hospice palliative care system within HNHB * pain and symptom management and education information * information about organizations and agencies providing palliative care services * information about palliative care issues, trends and research * links with local community partners
