DescriptionAdministers the Ontario Disability Support Program which is made up of two components: an income support program for people with disabilities who are in financial need and an employment support program that recognizes many people with disabilities can and do want to work. In order to be eligible for ODSP benefits the applicant must live in Ontario, be 18 years of age or older, be in financial need and have assets no greater than limits set for the program.  
Extended Health Benefit may help with the cost of prescription drugs, basic dental care, vision care, hearing aids, medical supplies, transportation to medical appointments, assistive devices including hearing aids. If someone does not qualify for ODSP income support due to excess income from other sources, they may qualify for the Extended Health Benefit.

EligibilityAges: 18 year(s) and up 
 
Resident of Ontario in financial need and certified as disabled by the Disability Adjudication Unit of the Ministry of Community and Social Services. Applicants who receive CPP Disability or QPP Disability, are 65 or older but do not qualify for Old Age Security, or live in certain types of institutions such as a mental health facility or a home for people with developmental disabilities only need the financial test.

ApplicationLocal intake process. Can call or walk-in for on-site intake with a client service representative. Can also be referred directly from Ontario Works. 
 
Applicants can also apply online for Social Assistance. A screening section can tell potential applicants whether or not they might be eligible before they start the application process.
