DescriptionCommunity Care Access Centres (CCACs) are the local point of access to community-based health care services. CCACs provide access to government-funded in-home health care (nursing, personal support, physiotherapy, occupational therapy, speech therapy and nutrition) and community services, and placement to long-term care homes. Also provide information about local community support service agencies, and we can link people to these agencies. Anyone can make a referral to a CCAC - a person requiring service, a family member, caregiver, friend, physician, or other health care professional. Work in partnership with clients, families, care providers, hospitals, long-term care homes and others to ensure that the right care is available at the right time and place. 
 
The Hamilton Niagara Haldimand Brant (HNHB) CCAC Branch offices are located in Hamilton, St Catharines, Simcoe, Brantford and Burlington. The head office is located in Brantford.

EligibilityPeople of all ages may receive services. You must be a resident of Ontario, have a valid Ontario health card, be able to receive care safely at home. Some clients may receive care in other settings such as at school or at a community based nursing clinic. Clients are assessed by CCAC staff using guidelines provided by the Ministry of Health and Long-Term Care.  
 
School Health Support Services are available province wide to children 21 years of age or younger with special needs who are registered with the Ontario Ministry of Education. Children must have a valid Health Card and be residents of Ontario needing special health services in the school setting. Children must attend a publicly funded or private school or receive satisfactory instruction in the home. Additional eligibility criteria will be assessed upon initial intake.

- Is your father no longer safe living alone and ready to move into a long-term care home?
