DescriptionSeniors Community Programs operates a range of programs for Niagara's older population. Assistance may be provided through information and support for specialized programs and services to help enrich lives and enable people to remain in their homes longer.

RESPITE COMPANION program provides a quality in-home respite service for persons with dementia and their caregivers. Respite Companions are trained to provide supervision, sensory stimulation, companionship and meaningful activity to individuals with memory loss, in their own homes, while offering relief to the primary caregivers. Personal care activity in not within scope. Service hours are variable - days, evenings, weekends and are on a scheduled basis.
Assisted Living Services -SUPPORTIVE HOUSING is a community-based alternative to long-term care for eligible seniors. The emphasis is on maintaining independence through the provision of assisted services within the apartment suite. Assistance with everyday non-medical activities such as personal care & medication prompts, security checks, attendant care, home-making support (i.e. light housekeeping), and life enrichment activities can be provided. Access and Eligibility for service is determined through CCAC. Seniors who are eligible for services, request and direct the required services including how and when they should be provided. Tenants residing within Deer Park Suites enter into a landlord /tenant agreement that is outside the provision of assisted living services.
CLIENT INTERVENTION AND ASSISTANCE Provides service support in stabilizing at risk situations. Community workers will conduct a home visit and through a formal assessment will identify needs and with consent, establish a plan of service support. Sharing of information about available services, initiating service referrals and monitoring the effectiveness of service supports helps a senior to preserve their independence. Will also investigate non-emergency allegations of abuse, neglect of a caregiver, self-neglect, isolation, safety and security.
