DescriptionThe Best Foot Forward program provides: 
 
* Standardized foot assessment and education at LHIN Chronic Kidney Disease (dialysis) centres 
* Nursing and chiropody foot care clinics at dialysis locations (Niagara, Hamilton, and Brantford) 
* Nursing foot care services available in the home for patients not able to attend the foot care clinic 
* Process for referral into the program will be through the Chronic Kidney Disease programs and participating dialysis centres 
 
EligibilityAges: 55 year(s) and up 
 
Seniors 55+ who have been diagnosed with diabetes and kidney failure and are unable to adequately manage their foot care (as there is a higher risk of foot ulcer with renal failure and diabetes)
