Description	Health Care Connect (HCC) is a brand new and innovative, made-in-Ontario program designed to helping Ontarians without a family health care provider find one in their community.    * HCC will not assist in cases where caller already has a family doctor even if they are out of town  * HCC takes both Emergency and Non-emergency cases  * Priority given to those with the most pressing need for family health care    Ontarians without a family health care provider can call the toll free number to register for the Health Care Connect patient database. The option to register online is also available at: www.ontario.ca/healthcareoptions.    Once registered with Health Care Connect, a nurse (called a Care Connector) will be assigned to help find a health care provider in the the client's local area. This may be a physician or a nurse practitioner. The Care Connector assigned will be the main point of contact with the program. Contact information for the Care Connector will be received after program registration.
